Print and Complete:
- Medication Consent Form Print and complete this form if your child has medication will be stored in the school office and administered by designated school staff.
- Lead Screening Form Print and complete this form if your child is UNDER the age of 6 and mail it to the address on the form.
- Immunization Record– required for Kindergarten and all new students (please mail to school, email to mwagoner@mirowitz.org or bring by the office). No Mirowitz form required, please submit this information from your doctor. THIS IS DUE AUGUST 1.
